New User Process
To receive an end-user license, you must first complete the steps below.
Please submit an account request form so the HMIS team knows how to set up your account on the HMIS site.
The end-user license agreement must be signed before the HMIS site can be accessed by an end-user.
Once you submit an account request form and sign the end-user license agreement, complete the annual training and review.
Schedule a follow up meeting to continue HMIS training and learn what the next steps are.
